please download the form above, complete and return it to your
nearest branch who will then contact you regarding your
application.
Please
note that there is a weight limit of up to 75kgs to ride at
SARDA and this is dependant on the rider's disability. If you
are unsure about your eligibility in this regard please contact
your nearest branch for more information.
